Adding another leg to our battle against COVID19, the Government of India has introduced, a promising anti-COVID19 drug called 2-deoxy-D-glucose (2-DG). The drug has been developed by the Institute of Nuclear Medicine and Allied Sciences (INMAS), a lab of Defence Research and Development Organisation (DRDO), in collaboration with Dr Reddy’s Laboratories (DRL), Hyderabad.The drug was approved for emergency use as an adjunct therapy in moderate to severe COVID19 patients by the Drugs Controller General of India (DCGI) early this month.
WHAT IS 2-DEOXY-D-GLUCOSE (2-DG)?
2-Deoxy-D-Glucose drug has historically been extensively tested for treating Cancer but is so far an unapproved drug. However, for COVID19, the drug shows promise. The drug comes in powder form in a sachet, which is taken orally by dissolving it in water. It accumulates in the virus-infected cells and prevents virus growth by stopping viral synthesis and energy production. Its selective accumulation in virally infected cells makes this drug unique.The Institute of Nuclear Medicine and Allied Sciences (INMAS), a lab of the DRDO, in collaboration with Dr Reddy’s Laboratories (DRL), Hyderabad, has been studying this drug, in the context of radiation therapy for Cancer.
The basic mechanism of the drug involves inhibiting glycolysis or one of the ways in which cells break down glucose for energy. This approach while used to starve and kill Cancer cells could also work in inhibiting virus cells too, that were dependent on glycolysis for replication.When the pandemic broke out in India, INMAS, DRDO and DRL switched its effort to explore the possibilities to use this drug to defeat COVID19. Tests at the Centre for Cellular and Molecular Biology, Hyderabad, last year indicated that the drug demonstrably killed virus cells after which it progressed to trials in people.
2-DEOXY-D-GLUCOSE DRUG CLINICAL TRIAL: GATHERING CLINICAL EVIDENCE AND EFFICACY
In April 2020, INMAS started experimental examinations in Hyderabad with the Centre for Cellular and Molecular Biology (CCMB). After which, the Central Drug Standard Control Organisation (CDSCO) and the Drug Controller General of India (DCGI) jointly granted permission for Phase-2 trials in May 2020.Between May to October 2020, the institute started initial trials on how COVID19 patients would respond to the drug. The drug worked well with no side effects and the patients recovered quickly. Further on, the Phase 3 clinical trial was conducted between November 2020 – March 2021 in Delhi, UP, Bengal, Gujarat, Maharashtra, Telangana, Andhra, Tamil Nadu, and Karnataka. The results were favourable.
THE EFFICACY FACTOR:
Clinical trial results have shown that this molecule helps in faster recovery of hospitalised patients and reduces supplemental oxygen dependence. The drug will be of immense benefit to the people suffering from COVID19.As per clinical data for efficacy trends, the patients treated with the 2-DG drug showed faster symptomatic cure than Standard of Care (SoC) on various endpoints. A significantly higher proportion of patients improved symptomatically and became free from supplemental oxygen dependence (42% vs 31%) by day-3 in comparison to SoC, indicating an early relief from Oxygen therapy/dependence.
ACTING WITH RESPONSIBILITY:
Approval and availability of this drug have come at a time when our nation is grappling to cope with the impact of the devastating second wave of COVID19, which has stressed our infrastructure &resources to its limit. I see a ray of hope, that with the availability of this drug we may be able to reduce the burden of COVID19 and save as many lives as possible.Having said that, precaution is a must. No COVID19 medication should be taken without a doctor’s prescription. Moreover, hoarding of drugs is a crime and as responsible citizens of the nation, we must ensure that these medicines should be available and accessible to those who need it.

Alongside COVID19, India now battles Mucormycosis or ‘black fungus’ – a rare, yet lethal infection. The latest NPPA report states that the government has revamped up the procurement and availability of Intravenous (IV) Amphotericin B (lipid formulation) which is an anti-fungal medication to fight the black fungus. The supply position is expected to improve with extra imports of this drug and increase in its production domestically. Moreover, states have been requested to put in place a mechanism for equitable distribution of supplies amongst Government, Private hospitals and health care agencies. Given thus, the Maharashtra government set up a taskforce as the state is witnessing a three to four times surge in Mucormycosis.
WHY IS THE BLACK FUNGUS SO DANGEROUS?
Mucormycosis is a serious but rare fungal infection caused by a group of mold called Mucormycetes. It causes blindness, organ dysfunction, loss of body tissue, and even death if not treated on time. Therefore, active screening and early intervention are a must. It can affect areas from where it can be inhaled through the nose/ sinus/ lung. If it enters from a wound or skin, it can cause a local infection. But if it enters from the sinus, it can involve the eyes and brain. Poor control in Diabetes is the biggest cause for the rising number especially post-COVID. Also, patients with immune-compromised systems or those who have undergone transplants or people who are on immunosuppressants or on a ventilator for a long period of time are at higher risk.
According to the taskforce advisory, infection with Mucormycosis should be suspected when there is:
Sinusitis – nasal blockage or congestion, nasal discharge (blackish/ bloody)
Local pain on the cheekbone, one-sided facial pain, numbness or swelling
Blackish discoloration over the bridge of nose/palate
Loosening of teeth, jaw involvement
Blurred or double vision with pain
Thrombosis, necrosis, skin lesion
Chest pain, pleural effusion, worsening of respiratory symptoms
WHY AMPHOTERICIN B IS EFFECTIVE?
Treatment of Mucormycosis involves a combination of surgical debridement of involved tissues and antifungal therapy. Intravenous (IV) Amphotericin-B is part of antifungal therapy. The usual starting dose is 5 mg/kg daily of Liposomal Amphotericin-B or Amphotericin-B Lipid Complex, and many clinicians will increase the dose up as high as 10 mg/kg daily to control this infection. But this cannot be taken without the doctor’s prescription and is to be administered only in an hospital setting. The duration of this treatment is 3wks, followed by oral medicines for another 3 to 6wks.
SCREENING SHOULD BE RAMPED UP TOO:
Screening for the black Fungus is a crucial step for early intervention. Also, elimination of predisposing factors for infection, such as Hyperglycemia, Metabolic Acidosis, immunosuppressive drugs, and Neutropenia, is also critical. Due to the difficulties in establishing a definitive diagnosis, many patients will be empirically treated for Mucormycosis because they have risk factors for infection and positive cultures and/ or compatible clinical syndromes. But this again will depend on the patient’s case.
Right now, the nation is battling the pandemic on several fronts. The new worry about the black fungus is only perpetuating the problem. While the governing authorities and healthcare providers have braced themselves to confront the problem, it is the duty of the citizens as well to protect themselves and their loved ones.
CITIZENS SHOULD PROTECT THEMSELVES:
The most effective way to prevent the black fungus is to wear a mask while venturing out, especially to gardens, dusty areas, where there is garbage bumping or food rotting. Secondly, wear long clothes and have less exposure to skin. Most importantly for diabetic and other immune-compromised people, controlling sugar levels and blood glucose is a must. People who are prescribed steroids should be constantly monitored and the dosage should be reduced in consultation with your doctor. Importantly, steroids must only be taken after consulting with your doctor – do not semt medicate.

When the pandemic first broke out, re-purposing broad-spectrum antiviral agents were deemed ‘safe-in-man’ through testing on early phase clinical trials for COVID19. Thereafter, many drugs were put to use availing the emergency authorization with government agencies. Once such drug was Ivermectin, which is mainly prescribed for patients with mild to moderate infection. With the second wave raging upon us, there was a huge surge in demand for these drugs.
WHO GUIDELINES
Recently, WHO issued guidelines saying Ivermectin should only be used on COVID19 patients in clinical trial settings. The UN health agency said there was a “very low certainty of evidence” on Ivermectin’s effects on mortality, need for hospital admission and getting rid of the virus from the body.After which, many state governments in India have rejected its use for COVID19 patients.The mechanism of action has not been explained here; this has caused confusion and worry among patients and providers alike. But let me explain the truth behind it; there is enough evidence that show the effectiveness of the drug. A study published in the American Journal of Therapeutics clearly mentions of a large majority of randomized and observational controlled trials of Ivermectin that reported repeatedly on the large magnitude improvements in clinical outcomes. Numerous prophylaxis trials demonstrate that regular Ivermectin use leads to large reductions in transmission. Moreover, a comprehensive review of the available efficacy data as of December 12, 2020, taken from in-vitro, animal, clinical, and real-world studies. all show the above impacts of Ivermectin in COVID19!
Here are few points worth considering:
¾ Since 2012, multiple in-vitro studies have demonstrated that Ivermectin inhibits the replication of many viruses, including Influenza, Zika, Dengue, and others
¾ Ivermectin inhibits SARS-CoV-2 replication and binding to host tissue through several observed and proposed mechanisms
¾ Ivermectin has anti-inflammatory properties that prevents viral replication
¾ Clinical studies of the efficacy of Ivermectin in hospitalized patients show favorableoutcomes
¾ Exposure prophylaxis studies that showcase Ivermectin’s ability to prevent transmission of COVID19
¾ Ivermectin hastens recovery and prevents deterioration in patients with mild to moderate disease, treated early after symptoms
¾ The World Health Organization has long included Ivermectin on its “List of Essential Medicines”
¾ However, there are studies that suggest the need for controlled studies to better test efficacy in Vexing Syndrome
Having said, that anything used in excess or abused is harmful to health. Providers need to careful while prescribing these drugs as they are still under review. The best way is to be wise and not let panic take precedence over our logical reasoning. Most of these repurposed drugs have their own pros and cons when used for COVID19, this needs to be explain to the patients while prescribing it.We have to be cautious but wise too. Let us all make an informed choice an ensure that we all come out of this crisis stronger and more empowered.

April was a chaotic month. What we saw in the past four weeks was no less than an episode of a web series that documented a myriad of emotions, mainly on the sad side of human life. Delhi became the COVID capital of the World with almost 40,000 positive cases of COVID-19 infection on a daily basis. The positivity rate crossed 30% and it resulted in a catastrophic disaster. The emergency rooms of all hospitals were choked with the rain of COVID patients, it was a healthcare disaster. Overburdened front line workers and little room to accomodate everyone, a dire situation turned up of lack of oxygen supplies that killed hundreds of innocent lives.
“I must have said, No and Sorry to so many of the people who approached me in the last month”, says Dr Charu Dutt Arora, a Home Care-Intensive care Covid expert physician. Many people think that Home Isolation just means monitoring Spo2 (Oxygen saturation) for mild patients at home. However, this time we witnessed that Dr Charu with his team managed more than 100 critical patients at home. “We tried to treat these patients, manage their vitals, provide complete nursing support and successfully avoided more than 95% of these to require hospital admission”, says Dr Charu with a humbling smile.
One of his patients was Mrs Thapar, a senior citizen living in the affluent residential society of Faridabad. When she found on the 20th April that she is COVID positive, she isolated herself and consulted Dr Arora. But not did she know that it would be less than 48 hours that her oxygen levels would start to fall and lead to breathlessness. “My aim is not just to provide Oxygen support at home, rather avoiding the situation to turn into Cytokine storm and take care of her blood coagulation issues”, comments Dr Charu on this particular case. The lady’s CT scan severity score was 18/25 (severe pneumonia suggesting almost 60-65% of lung involvement). With an army of nursing support, continuous oxygen management and skilful treatment, she came out as a COVID survivor last week.
Home ICU has proven to be a boon bridge between a patient’s home and hospital. It has proven to be a successful healthcare vertical that has decreased the load on hospitals and provided comfort and personalised care to critical patients suffering from COVID. Another successful case managed by him through HOME ICU set up is of Mr Singh, a young 34 year old man in Palwal. He was on NIV (non-invasive ventilator) support for more than two weeks and with the holistic medical approach, he met Dr Arora yesterday in his follow up the outpatient room.
“Working 16-18 hours daily has been a very tough ordeal, where I go home just to sleep and have my breakfast and leave for work. But managing patients remotely and planning their treatment for successful outcomes takes care of all my stress. I believe strongly in the nursing staff who follow the guidelines meticulously and that’s what do wonders in the end. It’s all about teamwork, after all”, adds Dr Arora.

* Your series Hindmata is doing good, tell us a little more about your character?
I play the protagonist Samaira Jal in Hindmata. She is a South Bombay bred fashion designer who comes from money and lives in her own little bubble. And she is oblivious to a life outside her world and therefore naive in her ignorance. She is always putting on a show to hide her own shortcomings and yet is almost endearingly delusional in her self-belief. She has a good heart even though she says the most outrageous things and the show is a coming of age tale that sees her find her voice by the end of it. As she is thrown into this completely alien world of inmates in Hindmata Women’s prison, she discovers who she is and eventually stands up with and for the prisoners.
* When did you decide that you want to become an actor?
I can’t put it down to a single moment because my journey has been very organic. I started with workshops and classes in school/college performances etc. One thing led to another and in trying to find me, I found myself a career. But yes a more conscious pursuit started after having finished my Engineering degree. Before that, I didn’t know it was a career option. No one in my family has ever been in the performing arts so I am an alone crusader of sorts from my clan making my place in the industry.
* Are you flexible enough to work on short notice as a stand-in? How quickly can you assume a new role?
The flexibility would depend on my schedule but if I am not locked in for another project, I am pretty quick with getting into character. I am fairly instinctive as an actor so that helps. Also I am naturally a very curious person so I also try and constantly acquire different skill sets.I am a trained actor, dancer, and singer. I train in martial arts and swimming. I learned how to ride a bike for a project. So I try to give myself as manyexperiences as possible which can only help in my work.
* Describe your most challenging role to date.
I have been very conscious in picking extremely different characters with every project and each of those comes with its own challenges. And I think my character in Hindmata was challenging because I had to tow the fine line between saying all these outrageous things and yet making it believable enough for peopleto buy that a person like this could exist. I also had to craft a different body language for Samaira and then create very subtle shifts in demeanor as she changes as a person during the course of the show.
* What suggestion would you give to young aspiring actors?
I would say train yourself and be very clear about why you want to do this. If you are just chasing the glamour and glitz, you will very quickly become disillusioned because this profession will challenge you in every way possible…emotionally, physically, intellectually, and spiritually.
* The entertainment industry is changing, where do you see yourself in the next 5 years?
The industry is definitely changing and gradually becoming more of a meritocracy than it has been ever before. In 5 years I see myself continuing to work with the best and being one of the best in the industry. I am also looking to do a lot more International work going forward.
* You will be seen in Brahmastra along with Ranbir Kapoor and Alia Bhatt. Tell us more about the project and your character.
I am not at liberty to tell you much about either of those unfortunately but I can tell you that it is a very fun part and I am very grateful to be working with the best in the industry. I signed the film 3 years ago before Helicopter Eela even released. It has been a great learning experience and one that I will carry with me to hopefully even bigger and better projects.
* Anything in works that you can talk about? Any exciting new projects?
I have another film that should be out later this year; A very quirky film and one that I can’t wait to tell you guys more about. I can tell you that I play a fiery journalist in it and that shoot has been one of the most creatively satisfying experiences of my life so far. I have also started work on my nextoriginal song. Will be making an announcement soon about both of these.
About Rashi Mal
Rashi started her acting journey on the stage working with and being trained by some of the best mentors in theatre, including Jeff Goldberg, Makrand Deshpande, Sunil Shanbag, and Divya Palat. Rashi got her diploma in Method Acting from the Jeff Goldberg Studio and has also trained at the New York film academy and London academy of music and dramatic art. She has worked on several web series including Pyaar Actually with YRF, Akarsh Khurana’s Boygiri, and A.I.SHA (that has seen a lot of International success and airs on the biggest TV Channel, TF1, in France)
She has shot for over 40 commercials for names like Close Up, Volkswagen, Nivea, KFC, Myntra. Multi-talented as she is, Rashi has also done multiple music projects. From singing to trying her hand at rapping in the film ‘Dear Maya’ starring Manisha Koirala. She’s a trained singer in western classical based singing with opera singer Deirdre Lobo (renowned concert artist and vocal coach based in California) and is currently training with Ms. Samantha Edwards who has also coached big names like Priyanka Chopra and Ananya Birla.

In addition, Yas Island’s theme parks and attractions were awarded the ‘Go Safe’ certification by the Department of Culture and Tourism – Abu Dhabi, including Ferrari World Abu Dhabi, Yas Waterworld, Warner Bros. World™ Abu Dhabi, CLYMB™ Abu Dhabi earlier in the year, Yas Marina Circuit, Yas Mall, as well as W Abu Dhabi – Yas Island – the first hotel in the UAE to receive the accreditation – and all Yas Plaza hotels. The certification is one of 25 health and safety accreditations received by the destination, including the coveted WTTC ‘Safe Travels’ stamp by the World Travel & Tourism Council in August 2020 – the first destination in Abu Dhabi to receive the international honor.
Yas Island also welcomed Hilton Abu Dhabi Yas Island in February 2021, located in the vibrant Yas Bay development. The property is Yas Island’s first five-star family hotel with beach, offering 525 spacious rooms and suites, and also includes a beach club, provides easy access to The Waterfront’s numerous entertainment and nightlife attractions, which include the pier, 37 F&B outlets and 19 retail outlets.Hilton Abu Dhabi Yas Island is part of Miral’s AED 4 billion investment into the overall AED 12 billion masterplan of Yas Bay which will feature three distinct areas across 14 million square feet; The Waterfront, where Hilton Abu Dhabi Yas Island is located, The Residences, and Yas Creative Hub.
This year also saw the first live event at the all-new Etihad Arena, the region’s largest multi-purpose arena, with UFC Fight Island™ Triple Header, hosting a limited number of fans. A line-up of live events will be announced throughout the year following its success, with popular stand-up comedian, Michael McIntyre, scheduled to take the stage on July 16.
As the UAE enters the summer months, Yas Island will be introducing its popular ‘Kids Go Free’ packages across the destination for the ultimate summer break. 2020 marked Ferrari World Abu Dhabi’s 10th anniversary, and the award-winning theme parked celebrated by launching two new first-of-their-kind experiences – the Roof Walk and Zip Line. Guests can scale the Park’s signature red roof and take in spectacular vista views of Yas Island and zip from the heart of Ferrari World Abu Dhabi through the Flying Aces roller coaster loop.

To date, Yas Island has introduced numerous safety measures such as the widely commended ‘Yas Wellness Program’ and the ‘#TogetherAtYas’ platform, an online platform that offers guests a guide to the comprehensive safety measures on Yas Island and is a one-stop shop for all the responsible and fun experiences on Yas Island.
Launched as part of ‘#TogetherAtYas’, the Yas Wellness Program is a comprehensive wellness program that was created in addition to the island’s enhanced hygiene and social distancing measures, granting guests direct access to a dedicated team of Yas Wellness ambassadors, a team of specially trained individuals who are able to answer guests’ queries on the health and safety measures in place at the reopened Yas theme parks, hotels and attractions, as well as answers questions regarding openings and closures.
